By virtue of someone's past experience in helming a ship, or bringing a ship into port, mooring the ship to berth, which is by all means the most complex of all seamanship, to steer a ship in by rudder and engine to a slot only with metres ahead and astern, with these expertise, the harbour pilot is granted his customary Captain status by master mariners whom recognize the complexity of the task.
Firstly for one whom has yet seen the rest of the world and the many ships whom have "SUITABLY QUALIFIED MASTER MARINERS" whom dun give a fuck about your life and death, whom will menindas you and treat u like the maggot u are.. Maybe you will want to revisit that thought when you look at the lecturers who actually give a fuck whether you pass or fail in your institution, and call them Capt.
Outside of a maritime vessel, people still call them Capt in respect of their contributions, and past skills and experience.

When you do, you will see that, Companies put the title Captain for you, not the Government in a maritime setting.
